How we can help you be active and healthy
How we support you
At Healthy Ageing Norfolk, we know that the best way to truly understand your needs is by meeting you in person. That’s why, following an initial phone call, a qualified Support Worker may visit you at home. These visits give us the opportunity to get to know you better, talk through your goals and wishes, and check for any risks that could be linked to frailty or changes in your health.
Your safety and comfort are always our priority. All members of our team are DBS-checked, and we’ll arrange a visit at a time that suits you. If you’d like a family member, friend, or carer to be present, we’ll make sure the appointment works for them too.
During the visit, we’ll take time to have a friendly, open conversation about a range of areas that may affect your health and wellbeing.

Vision & Hearing
We’ll ask when you last had an eye or hearing test and carry out a simple check if needed. Good vision and hearing are vital for staying independent and preventing falls. If you need updated tests or aids, we can help you arrange them.
Home & Footwear Safety
We’ll help you spot potential hazards in your home, such as safe footwear, poor lighting, loose rugs, or clutter. If needed, we can connect you with local handyperson services or help arrange home adaptations to make your environment safer.
Hydration & Nutrition
Drinking too much alcohol and can have an impact on your health in later life, and by staying within the low risk drinking guidelines will help you to have healthy overall lifestyle.
Health & Medication
We’ll understand any conditions you live with, symptoms that affect you and may increase your risks of falls. We’ll discuss any side-effects from your medication and support you with any health or care appointments.
Life & Social
Loneliness, housing, money worries, or relationship difficulties can all affect your health. We’ll explore whether you need help in any of these areas.
Stability & Strength
We will ask about any previous falls or fears of falling. We’ll check your balance, mobility, and strength. We will complete some sitting/walking tests to see what exercises can help you.
